Right to Erasure
You can ask Crystal Health Group to erase personal information we hold about you. This is known as the right to erasure or the “right to be forgotten”. Requests are normally free of charge.
How to make a request
You can contact us by:
- Email: info@crystal-health.co.uk
- Phone: 0800 988 7107
- Post: Quality and Compliance Team, Crystal Health Group, The Old Chapel, 53 Peel Street, Eccles, Manchester, M30 0NG
Tell us what information you would like erased. A case reference or relevant dates can help us find it, if you have them. If you also want us to arrange destruction of a retained biological sample, please tell us.
You do not need to complete a form or explain your personal reasons for making the request. If you would find a form helpful, please ask us for one.
What happens next?
We will acknowledge your request and normally respond within one calendar month. Where necessary, we may ask for information to confirm your identity or your authority to act for someone else. If identity checks are needed, the response period begins once we have sufficient information to confirm who you are.
We will assess your request and explain the outcome. Erasure applies in certain circumstances, and we may need to retain some information, for example to meet a legal obligation or deal with legal claims. If we cannot erase all the information requested, we will explain why.
For complex requests, or where you make several requests, we may extend the response period by up to two further months. We will explain why within the initial month.
If another organisation, such as your employer or a healthcare provider, is responsible for some of the information, we will explain who should handle that part of your request.
Questions or concerns?
Please contact us using the details above if you need help or have concerns about how we have handled your request.
