Subject Access Request

Under the UK General Data Protection Regulation (UK GDPR), you have the right to access personal data that Crystal Health Group holds about you.

To make a formal request, please complete our Subject Access Request (SAR) form (see button link below) and return it to us by post. For data protection and identity verification purposes, we also require a photocopy of valid photographic ID and a wet (physical) signature on the form.

   Fill in all sections, including your signature.
3. Provide Proof of Identity
   Enclose a clear photocopy of one of the following:
   > Passport
   > Driving licence
   > Official ID card
   
4. Post Your Completed Form & ID Copy To:

Once we receive your completed SAR form and valid ID, we will:

• Acknowledge receipt within 5 working days
• Begin processing your request
• Provide a response within one calendar month

If your request is complex, we may extend the timeframe by up to two months — we will notify you if this applies.

To ensure your personal data remains secure, we require a physical signature and ID verification. This process helps prevent unauthorised access to sensitive or health-related data we hold.
